I'm not sure I'm willing to play D&D with anyone who hasn't read this excellent tome of sheer awesomeness. This book actually makes a real attempt at tearing the so-called economy and social demographics of the typical D&D world to rubble, in a hilariously ironic way.

Nobody really expects any RP world to be watertight, but I never really looked hard at the holes before.

Similar to the Dungeonmicon, the Tome of Fiends talks primarily about the role of EVIL in D&D. Precisely, it talks about how evil CAN relate to a world in general, as well, as a metaphysical force that Gods will Reward you for following.

Both these articles contain several new takes on classes, almost of all of which have instant appeal. I'm pretty sure most of us would love to try out at least one of these things somewhere.

I was gonna post more, but really, I think this is more of a case of reading up and taking away from those what you want to.

I just read that entire thing. It took me an hour and a half, but I did it.
Now to go make a rediculously stupid dungeon that's built like a modern high rise, because that's what my players are expecting. Even though it's underground and has no use for windows, it's going to have windows and stuff, with no apparent use. (Hopefully this will give my players incentive to roleplay a bit.) Maybe some morale boosters, like random flags and standards all over the place. And a kitchen. One of those can't hurt. Maybe a cool choke point about half way through, where there's a 5x40 hallway that has a 20 foot deep hidden pit in it, and the walls ooze hot tar, but all of the troubles can be avoided with a DC20 reflex save.

...wait. You can just dimensional anchor Efreeti to conjure up items of 15000 gold or less and sell them? Hmm....
