Surraruthru activates his Boots of Swift Passage, teleporting to the furthest away from the bloated thing and putting as many things between himself and it as he can, and struggling against the hands as best he can.

Boots of Swift Passage, 10,000gp: 20 foot teleportation as a move action 5/day.

No need for CMB, the teleportation gets you out of grapple.

You teleport past the brains, using them for cover. The hands all writhe and begin to shake as the bloated nightmare stands. It quakes visibly, the chains lashing out at you like whips.

The brains'll give you cover, which grants +4 to AC, which will boost your CMD.

The chains smash through the brains, which howl even louder in your mind. One of them is knocked away, torn in half and falling to the ground. Grasping hands tear it apart as the other half gushes fluids out, but it doesn't stop the chains. They grab you again, wrapping all around you.

Go. You're grappled by the chains again.

Yes, move+standard. The boots are a move action and portal is a standard. If you are, make the Intelligence check, this time with a +2 circumstance bonus.

A portal appears, just as desired. You teleport before it and stumble through, leaving the nightmare behind.

You appear in a dark, barely lit place. Walls of wood, bookshelves. They're filled with books, dim light coming from nowhere and everywhere. Twisting passages through the walls are about, all over. As you get your bearings and catch your breath, you hear screams. Ebony's screaming, echoing and distant, but distinctly her.

It's hard to track but you do the best you can. You begin to follow, down a twisting, dark corridor. As you go, you can hear Ebony screaming.

"Alicia! Alicia! Alicia!" Over and over like a chant, calling her name with a celestial accent. You reach a turn and go right based on the direction the noise is, then Ebony screams again. Even louder, agonized. A screaming sob follows, "I will fear no evil, I will fear no evil, I-" Another scream, and a wet, tearing sound follows as you hurry along.

Another intersection, going left this time as you follow.

Chance to react or do anything, or keep going as you please.
