It's the third layer of Acheron. Lots of uneven shapes and collisions there, so it's relatively lightly inhabited. It's still a battlefield but more of a brutal one. Fiends cavort there and the Blood War is fought, as it often is on the lower planes, but it's mostly general nastiness.

Are there any groups or a single rumored entity that might match up with what we've seen of Guardians in the past?

Nothing that leaps out as such, no.

Which forces hold sway there?

[Anastasia]

Mostly devils and warlords. It's a nasty brutish place in the shadow of Baator. A lot of various evil deities have some influence here, but it's a bit wide open in that aspect. Used for that freedom for all sorts of nastiness meant to be kept out of general circulation.
